2. Important Information About Your Enrollment

To apply for coverage, complete your enrollment form within 31 days of your eligibility date. If you apply after that date, or if you have been previously declined for LTD coverage offered through the Group Insurance Commission, you will be required to provide evidence of insurability in order to qualify for coverage. This will include a review of your overall medical health including routine, planned, unplanned or ongoing medical care or consultation, and may result in a declination of coverage.

SPECIAL ENROLLMENT: April 10, 2013 - June 14, 2013. No medical questions asked or need for Evidence of Insurability unless you have been previously declined.

4. How do I file a Long Term Disability Claim?

As an employee of The Commonwealth of Massachusetts, you may contact Unum directly to initiate a Long Term Disability claim. You should contact Unum 30 days prior to the end of your elimination period (90 days from your date of disability).

To initiate a claim, dial toll free 1-877-226-8620 and select option 1.

Once you have initiated your claim by calling Unum, a Long Term Disability claim package will be mailed to your home address within one (1) business day. The claim package will include the following:
  1. Cover Letter with instructions;
  2. Claimant Statement;
  3. Attending Physician’s Statement which must be completed by your doctor; and
  4. Employment Statement which must be completed by your Agency Coordinator

All paperwork should be submitted to the address or fax number indicated on the claim form.

When Unum receives all requested information, a claim number will be assigned and a Unum Disability Benefit Specialist will contact you directly within five (5) business days to establish next steps.

5. How do I check the status of my Long Term Disability claim?

You can check the status of your long term disability claim by calling 1-877-226-8620 (toll free). You may speak with a Contact Center representative directly or use our Interactive Voice Response (IVR) system once you have received your claim number in the mail. The IVR is available 24 hours a day and the Contact Center is open 8am to 10pm (EDT).

You can also check the status of your long term disability claim via the internet at www.unum.com. Your Unum Disability Benefits Specialist will provide detailed instructions in your initial letter.
